Job Overview

The Clinical Social Worker I is a licensed clinician that conducts psycho-social assessments and interventions with patients/families and serves as a liaison with multi-disciplinary care team and external community agencies to implement interventions.

Qualifications
  • Education: Master's degree in Social Work
  • Experience: Equivalent of 1 years of full time work experience providing psycho-social assessment and interventions to patients and families.
  • Licensure(s): Licensed Social Worker (LSW) in the state of Colorado (CO)

Responsibilities

POPULATION SPECIFIC CARE

  • Neonate - <30 days
  • Infancy - >30 days to 1yr
  • Toddlers - >1yr to 3yrs
  • Pre-Schoolers - >3yrs to 5yrs
  • School age - >5yrs to 13yrs
  • Adolescent - >13yrs to 18yrs
  • Adult - >18yrs to 65yrs

ESSENTIAL FUNCTIONS

An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.

  1. Develops and implements service/treatment interventions that provide a variety of services to meet patient/family needs.
  2. Monitors progress, cooperation, and acceptance of services.
  3. May serve on department and hospital-wide committees as directed.
  4. Develops a therapeutic relationship with the patients and families to assist with the social, financial, and psychological problems associated with hospitalization and/or outpatient treatment.
  5. Develops and maintains relationships with peers within and outside of the department as well as community resources.
  6. Conducts initial needs assessment to gather information on the patient/family's medical, social, and financial history.
  7. Serves as a liaison with multi-disciplinary care teams and external community agencies to identify, implement, and monitor treatment goals.
  8. Facilitates staffing, care conferences, etc to coordinate care between the treatment team.
  9. Compiles and prepares case records, reports, and documents that comply with state and federal standards in providing case notes, treatment plans, and evaluations.
  10. Participate in on call and rotation.

SCOPE & LEVEL

Complexity: Falls between the entry and full performance/senior level. Employee primarily applies standardized practices.

Decision Making: Decisions or recommendations on non-standardized situations are limited to relating organizational policies to specific cases. Brings non-routine issues to supervisor. Problems that are not covered by guidelines or that are without precedent are taken up with the supervisor.

Communications: Contacts with team members, clients or the public where explanatory or interpretive information is exchanged, gathered, or presented and some degree of discretion and judgment are required within the parameters of the job function.

Supervision Received: Under normal supervision, within a standardized work situation, the employee performs duties common to the line of work without close supervision or detailed instruction. Work product is subject to continual review.

Salary Information

Annual Salary Range (Based on 40 hours worked per week): $63,317.59 to $94,976.38
Hourly Salary Range: $30.44 to $45.66
