The following positions within Inpatient Adolescent Psychiatry, Inpatient Adult Psychiatry, Psychiatric Emergency Services, CARES, and Outpatient Behavioral Health Services are offering the opportunity to enroll in a new program offering a cash incentive to recognize your work treating patients with substance use disorders. This program is sponsored through the ACCESS (Advancing Careers, Competencies, and Equity in Substance Treatment Services) grant. Employees must be >0.6 FTE to be eligible. 50% of the bonus will be received 90 days after signing the program contract.

* Registered Nurses (RNs) may earn up to $7,500 by December 2024
* Behavioral Health Technicians (BHTs) may earn up to $3,500 by December 2024
* Care Navigators may earn up to $3,500 by December 2024

Job Summary

Provide coaching, mentoring and leadership to care navigators. Assist patients with moving through the health care system in order to improve health outcomes. Provide outreach for quality improvement initiatives and cost effective use of available health care resources. Provide program oversight and development, training, and administrative duties. Identify learning opportunities for care navigators.

Essential Functions:

  • Ensure care navigator productivity and work quality (50%)
  • Create standard work and ensure processes are followed including outreach initiatives, care coordination, patient education and documentation. (10%)
  • Audit workflow processes and documentation to ensure compliance with program expectations. (10%)
  • Collaborate with clinics for new staff training on core competencies. Assume responsibility for coordinating ongoing training opportunities for staff to ensure development of staff skills. Update and train on new processes and workflows to reflect any changes in a timely manner. (10%)
  • Identify and obtain cultural and community resources for patients' needs. (5%)
  • Remain current on all job-related knowledge, technical skills, on-going education, and certification/registry requirements. Liaison with internal colleagues and community partners to ensure quality and competent services are provided and align with institutional initiatives and goals. (5%)
  • Be responsible for, but not limited to, hiring, terminating, performance management, coaching, supervision, fostering teamwork, training and development of one or more employees. (5%)
  • Collaborate with internal colleagues on tasks and responsibilities as capacity allows. (5%)


Education:

  • High School Diploma or GED required.


Work Experience:

  • 1-3 years as a Care Navigator - OR - a Bachelor's degree with 2 years of project/program management experience required.

Certifications:

  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association required.


Knowledge, Skills and Abilities:


1. Knowledge of basic care coordination and resources to reduce barriers for patients

2. Knowledge of internal and external community resources

3. Excellent customer service, critical thinking, and problem solving abilities

4. Must possess excellent communication skills

5. Must possess the ability to provide constructive feedback to care navigators and work with the management team


6. Use of Microsoft Word, Outlook, Excel, and Powerpoint

7. Knowledgeable in Epic

8. Listed on the Colorado Health Navigator Registry as meeting the credentialing processes needed by the Colorado Department of Public Health and Environment (CDPHE) (department paid training).
